If you have not registered your child to be COVID tested at Extera, now is the time to get them registered. Please click on the link to be directed to the registration page. To be prepared for registration, you will be asked to: upload pictures of Insurance card and ID like Driver’s License; Personal information about the student; Personal information about the parent, including an email address; Information about your Health Insurance and SSN; and Signing the Consent form for testing.
